Qualifications:
A graduate of a college or university with a bachelors degree in a human service field and has two years full-time, post-bachelors degree accumulated MH/DD/SA experience with the population served, or college graduate with a degree other than human service with four years of full time post graduate experience and/or a substance abuse professional who has two years of full-time, post bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling.
Responsibilities:
Document appropriate clinical information in medical record in a timely manner including but not limited to:
1. Monitoring the status of the recipients’ progress and the effectiveness of the strategies and interventions outlined in the PCP.
2. Submission of service notes within 24hrs of service delivery.
3. Offer coping skills to consumers served
4. Therapeutic interventions for the consumers in group setting
5. Crisis management
6. Completion and submission of all documentation required under the day treatment service definition
7. Ensure daily documentation is completed for shift (point sheets & behavioral tracking logs)
8. Provide transportation to consumers within the day treatment program as needed
9. Facilitation of therapeutic groups
10. Close out the shift by ensuring no materials are left out at the end of the shift, ensure the group areas are clean
11. Ensure incidents within the day treatment program are reported to supervisor immediately
12. Participate in administrative/clinical supervisions with supervisors at least once monthly
13. Follow through with recommendations from immediate supervisor

1. Complete the initial development and ongoing revision of the PCP and ensuring its implementation
2. Monitoring the status of the recipients’ progress and the effectiveness of the strategies and interventions outlined in the PCP.
3. Linkage to the most clinically appropriate and effective service(s)
4. Linkage to an alternative service when clinically indicated and functionally appropriate for the needs of the youth and family as determined by the CFT.
5. Development of youth and family coping skills
6. Therapeutic interventions for the individual, the family or both
7. Substance abuse interventions (if applicable)
8. Crisis management
9. Intensive case management
10. Complete NC Topps Assessments
11. Provide parenting skills training that builds effective communication and positive behavioral intervention strategies with the youth to help meet the PCP goals.
12. Work with the youth and his/her caregivers to implement home-based behavioral support plans.
13. Provide ongoing evaluation of the presenting psychiatric or addiction symptoms
14. Identify strengths that will aid in stabilizing the family unit, as well as identify barriers that may impede the development of skills necessary for functioning in the family and community
15. Provide psycho education to the youth, family and caregivers
16. Defuse the current crisis, evaluate its nature, and intervene to reduce the likelihood of a recurrence.
17. Provide first responder 24/7/365 availability for on-call assistance to consumers and families.
Qualifications:
The clinician shall have a doctor’s or master’s degree in social work from college or university accredited by the Council on Social Work, Education, or a field related to social work, such as sociology, psychology, education or counseling, with a student placement in providing casework services to children and families. One year experience in providing casework services to children and families may be substituted for a student placement. The Clinician position at a minimum must be qualified mental health professional.
Responsibilities:
1. Performs initial interviews and comprehensive clinical assessments.
2. Assists with developing service plans/person centered plans if applicable.
3. Submit comprehensive clinical assessments within 72hrs of the initiation date.
4. Implements, evaluates, and modifies service plans for all assigned consumers
5. Performs individual, family, group counseling, and crisis intervention for families and consumers.
6. Performs psycho-social assessments.
7. Assists with formulating and coordinates discharge and after care planning.
8. Attends all required clinical meetings.
9. Attends all assigned individual and group supervisions.
10. When required, provides supervision to program staff.
11. Participates in regular professional development in accordance with discipline and level of skill.
12. Is responsible for the proper and timely submission of documentation of all information required by the Department of Mental Health and Substance Abuse Services and Youth Extensions, Inc.
13. Actively coordinates case management, discharge, and aftercare planning and referral to agencies as necessary.
14. Assumes on-call responsibility as necessary.
15. Completes all other work related tasks assigned by the Program Director or the Clinical Director.
